State ID or license and Birth Certificate.. I would get a notorized statement from his/her parents saying they approve of the trip too.
Reply:You should have all of his personal information including phone number. You should have written consent from his parents. You will probably need his birth certificate and a picture ID or a passport. Good Luck
Reply:form of ID, including birth certificate...passport, personal info, letter from parents (power of Attorney) not just to cross the border, but also in case of any accident she can approve any medical attention that would be needed!
Reply:Probably birth certificate and letters of permission. Call the boarder people up and see.
Reply:There is a form that you can get from your doctor,.A medical release form that has all the information you will need in case of an emergency. Make sure that he has a picture ID High School Id is ok...or driver licence if he has that...But just be safe so you dont need to call on the doc for any thing..Take care
Reply:id get a permission slip signed from the parents... maybe noteerized afterwards.. that way you are protected if say anything happend. You personally go see the parents, your children and their children too sit down talk.. and ask if they would like to come with you. make sure all rules and regulations are set and understood
Reply:On a plane? They will definitely give you a really hard time at the airport unless its really clear who the kid belongs to and that their parents are clearly aware of the trip. You might want to seriously check with an experienced travel agent to get an official answer on that!
Reply:My mom took my friend and I to Jamaica for my 16th bday party. All she had to have was her passport, ID- normal routine.....

but i dont know if Canada differs or not
Reply:YOU NEED TO GET A NOTORIZED PERMIT FROM THE PARENTS STATETING THAT HE/SHE WILL BE UNDER YOUR SUPERVISION DURING THE TIME BEING.
